Due to the 2019-2020 outbreak of the novel Coronavirus, COVID-19, we are taking extra precautions with the intake of each client, health history review, and sanitation and disinfecting practices. Covid-19 form questions must be answered when booking appointments, and when filling this form out, we ask that integrity is in place.

Understand that you must follow the establishment’s guidelines:
Reschedule appointment if you are feeling unwell
No additional guest is allowed.
You are required to wear a mask at all times upon arrival and during the entire procedure, regardless if you are vaccinated.
Sanitize hands upon arrival.
Limit conversation during the procedure.
Time is precious and something that you can never get back. Lash Critic™ politely ask that you adhere to guidelines and etiquette.
CANCELLATION/RESCHEDULING POLICY: If you are 10 minutes late you may not get the full amount of eyelash extensions on but will be charged for the full price of the service. If you are 15minutes late your appt will be CANCELLED. For a late arrival, you MUST make communication PRIOR to your current appointment. Please be mindful of other clients that have appointments after yours. We will not push the next client's appointment (whom is on time) back to accommodate a late client. Therefore, the timer starts at your appointment.

NO SHOW/NO CALL/NO TEXT - APPOINTMENTS FORFEIT ANY PAYMENTS IMMEDIATELY AND WILL NEED TO MAKE ANOTHER FULL DEPOSIT TO MAKE ANOTHER APPOINTMENT. Again, your payment for that appointment will NOT be refunded. Clients will not be allowed to reschedule WITHOUT paying the FULL PAYMENT at time of rebooking. All fees will be charged to credit card on file.
NO SHOW- you don't bother to come at all
NO CALL- you failed to call OR you called BUT did NOT leave a voicemail stating your Name, date, and time of your appointment
NO TEXT/EMAIL- you failed to send text a message stating your name, date, and time of your appointment (I must acknowledge)
*Communication MUST be made PRIOR to appointment no less than 1hr before set appointment time.
RETAINER FEES range in price depending the service.
*Retainer fees are NON-REFUNDABLE! There is a ONE-TIME courtesy to reschedule an appointment if the cancellation is made 24 hours in advance; if not, there is a $25 rescheduling fee. After 2nd cancellation of rescheduled service, deposits are forfeited. The amount is deducted from the final cost. This rule does not cover No show/call/text so please read those guidelines.
PREPAIDS SERVICES *Due a number of "clients" not following the "no show/call/text policy and when attempting to charge their card on file and it declining... Prepaid services are now placed on some of the services. Prepaid services must be rescheduled within one week from original appointment. Prepaid Services are not charged a no show/call/text fee any monies will not be refunded. Prepaid Services will be charged a 15%-20% refund fee if such is requested.
CELL PHONE- Silence phones during your appointment and relax and enjoy your quiet time.
CHILDREN- We love children but due to the nature of the services provide children not allowed. Please make provide childcare for a min of 2hrs.
